New Patients

If you are a new patient to the practice we are now sending you a booking email which contains a link to complete your new patient questionnaire directly in your medical record. If you are unable to use this then please download the relevant new patient forms using the buttons below. These can be printed out and completed by hand then a scanned copy (or a photo from your phone) should be emailed to reception@stirlingpodiatry.com before your session. If you aren't able to do this then that is no problem, the podiatrist can take you through them during the appointment.

Treatment of Children (15 and under)

By law, those under the age of 16 require someone with the right of parental consent to be present during their treatments. This means that a parent (or someone granted the right of parental consent by the courts) must be present during treatment.

If this is not possible then we have created a special form that will allow another adult to bring the patient so long as the parent will be available by phone during treatment. We appreciate that this will be an inconvenience to some, but we have a professional responsibility to ensure that we carry out treatments in accordance with the law so we thank you for your understanding.

Failure to meet these conditions will regrettably result in the child’s treatment not going ahead.

For ease, all of our children’s forms are available for download here. We would strongly recommend that a parent accompanies the child to the first treatment but if this is not possible, please download and complete the new patient forms as well as the Absence of Parental Consent form and send those along to the treatment. When an email address was provided during the booking process, the new patient forms are also send out via an automated email at the point of booking.

Treatments for those with additional needs

We have a wide range of additional services that we can provide to those who require support. Please let us know what your needs are and we would be happy to help. Should you wish to speak to the podiatrist about your individual needs prior to the appointment, then please let the receptionist know when booking and we can arrange a call back.

So what extra ways can we help?

• Payment can be taken over the phone for those with difficulty handling money.
• We do not charge for late cancellations and failure to attend for patients with certain additional needs.
• Photos of staff can be downloaded from our website for communication books.
• Written overviews of treatment can be provided by email within 24 hours of the appointment.
• Care facility medical logs can be completed by our staff (if consent is in place).
• Phone, email and text reminders are all available on a schedule that suits your needs.

Please do let us know what your individual needs are and we will do our best to adapt the treatments to suit. We want to ensure that your visit to Stirling Podiatry is as comfortable as possible for you.

Cancellation policy

Should you no longer require your appointment please give us as much notice as possible. A 50% appointment charge will be applied if a patient fails to attend or less than 24 hours notice is given and the appointment cannot be filled. Repeat occurrences will result in higher charges being applied. Patients with additional needs such as dementia, Alzheimer's and learning disabilities are exempt from this policy.

Booked online? In the event you need to cancel your appointment your 50% deposit will be refunded to your card within 5 - 10 working days or will be transferred to your re-scheduled appointment date. If you have to give less than 24 hours notice of cancelling your appointment then your deposit will only be refunded in the event that we are able to fill your appointment space.
